Corrections & Amendments to Vital Records

Corrections and amendments to vital records are governed by Arizona statutes and administrative codes. The Office of Vital Records must abide by these laws when making amendments and corrections. The governing statutes are A.R.S. § 36-323 and A.R.S. § 36-337, and the administrative codes are A.A.C. R9-19-112.01 through 120. The process to make changes to a birth or death certificate varies depending on the type of change and how recently the record was registered.

A.R.S. § 36-323 and A.A.C. R9-19-116 restricts the list of people who may request corrections or amendments to birth certificates. Only the registrant (if the registrant is 18 years of age or older), the parent or legal guardian (if the registrant is under the age of 18) may submit a request to change the registrant’s name.

The Arizona Department of Health Services State Office of Vital Records no longer provides same day walk in customer service. The State Office of Vital Records provides the following correction or amendment services by appointment only. These services cannot be provided by the local county health department’s Office of Vital Records:

  • Adoptions (domestic) – all years
  • Corrections or amendments to birth records for births that occurred prior to 1997
  • Corrections or amendments to delayed birth records (all years)
  • Corrections or amendments to foreign-born birth records (all years)
  • Corrections or amendments to death records that occurred prior to 2008
  • Corrections or amendments to fetal death records that occurred prior to 2013
