Corrections & Amendments to Vital Records
Corrections and amendments to vital records are governed by Arizona statutes and administrative codes. The Office of Vital Records must abide by these laws when making amendments and corrections. The governing statutes are A.R.S. § 36-323 and A.R.S. § 36-337, and the administrative codes are Title 9, Chapter 19, Sections 112.01 - 120. The process to make changes to a birth or death certificate varies depending on the type of legal change needed and how recently the record was filed. You may want to call before coming to our office and find out what documentation will be required.
The Arizona Department of Health Services Office of Vital Records no longer provides same day walk in customer service. You may apply for a correction or amendment to the birth or death record by mail. For additional information or questions please call (602) 364-1300.
