When should I report?
Most disease reports
should be submitted within five (5) working days of diagnosis, treatment,
or detection. However, some conditions must be reported within 24
hours or only during outbreaks.

Who should receive the report?
Reports should be sent to
the local health
agency (county or tribal health department or Indian
Health Service Unit) by mail, telephone, or fax.
What information is required?
Reports should include
the patient’s name, telephone number, complete street address, date of
birth, race, sex, ethnicity, date of onset, diagnosis, date of diagnosis, laboratory results and date, name of reporter, and the reporter’s
telephone number and complete address. Submit mailed or faxed
