Case Definitions for Public Health Surveillance

This guide was designed for County Health Departments and Local Health Agencies in order to facilitate case identification and reporting of disease. Agencies should forward all reports of probable and confirmed cases meeting the Centers for Disease Control and Prevention (CDC) case definitions to the Arizona Department of Health Services (ADHS). 

Since the last revision was distributed in December 1995, the communicable disease rules and case definitions have undergone extensive revisions. Both ADHS and CDC have updated and added investigation forms. Links to all CDC and ADHS investigation forms are available from this website.

Questions about completing any investigations or forms can be directed to the Bureau of Epidemiology and Disease Control at 602-364-3860.
