Children's Rehabilitative Services
 

Children's Rehabilitative Services logoCRS provides family-centered medical treatment, rehabilitation, and related support services for children under age 21 with qualifying chronic and disabling conditions. Last year over 23,000 children and young adults received health care and related support services from the CRS program. CRS members receive care for their eligible conditions in multi-specialty interdisciplinary clinics, but do not receive general primary care services from the program. The majority of CRS members are also enrolled in an AHCCCS Health Plan, where they have a primary care physician who manages their care that is not related to their CRS-eligible condition. Arizona Department of
Health Services provides CRS services through a contract with APIPA-CRS.

To be eligible for CRS you must:
Have certain medical conditions;
Be under age 21;
Be a U.S. citizen or qualified alien; and
Live in Arizona.

To apply to APIPA-CRS you just need to fill out an Application/Referral FormPDF (English) or a Referencia/SolicitudPDF (Español).

Want to get involved? Join the CRS Parent Action Council (PAC). The PAC is a local council made up of parents of a child who is or was a CRS Member, adults who are or were CRS Members and the APIPA-CRS contractor. PAC members may also include professionals and members of advocacy groups. The PAC is a place where parents meet to share ideas and resources to make your health care better. PAC members share this information with APIPA-CRS and with the Arizona Department of Health Services, Office for Children with Special Health Care Needs. If you would like to attend a meeting or become involved in PAC activities, please contact Member Services at 1-866-275-5776.
