Immunization Data Reports (IDRs)
Arizona schools must submit an annual Immunization Data Report (IDR) on students enrolled in preschool, pre-k, kindergarten and sixth grades by November 15th each year. Childcare centers, small group childcare homes and Head Start programs must submit annual Immunization Data Reports on children born on or after October 1, 2009.
School Immunization Data Reports for 2014-2015 School Year
All Arizona schools with kindergarten and/or sixth grade must submit their annual Immunization Data Report through the Immunization Data Report (IDR) web application. The web application will be open for use by September 1, 2014. Please visit the application site to create a log-in for your school and review the questions included in the reports. Immunization Data Reports are due by November 15, 2014.
New Format for Childcare, Preschool, and Head Start IDR forms!
Master copies of forms may be printed, completed by hand, and submitted. The Excel-based, and EZ-Fill forms may be saved and completed on the computer, then printed for mailing. A copy of each Immunization Data Report page should be retained for the center's or school's records.
Large preschool or childcare programs that use software to track immunizations may wish to generate their own Immunization Data Reports. These reports are acceptable as long as they follow the Standards for 108/108B forms.
