During this time, Medical Marijuana Online Registry Applications will be unavailable. We apologize for the inconvenience this maintenance downtime may cause.   • The Arizona Department of Health Services will accept Dispensary Registration Certificate applications in late 2015. There will be more information forthcoming regarding which, and the number of, CHAAs that will be available.

Pursuant to Arizona Revised Statutes (ARS) §36-2807, ADHS has established a secure, password-protected, Web-based verification system for use by law enforcement and employers.

NOTE: Medical Marijuana Registry Identification cards are now printed with colored bars distinguishing each card type and a corresponding Quick Response (QR) Code on the back of the card:

  • Qualifying Patient – Yellow
  • Designated Caregiver – Red
  • Dispensary Agent – Blue
  • * Dispensary Member – Purple

ADHS will not be re-printing existing cards with the new color scheme and QR Code. Cardholders will receive this new card when and if a new card is issued.
