Arizona J-1 Visa Waiver Program

Service Commitment – Obligation Monitoring

After the start of the J-1 waiver obligation, the participant is required to submit documentation for services rendered at the approved service site. The service verification and encounter report formsPDF with notarized signatures must be submitted to ADHS quarterly within 10 days of each 90-day of completed service.

Consequences of Default

Employers
Any violations of the conditions of sponsorship may result in ineligibility to participate in the Arizona J-1 Visa Waiver Program for a period of two years.

Physicians
A physician is considered in default if, at any time, he or she does not meet the conditions of the Waiver Agreement. Any violations of the conditions of state sponsorship will be reported to the Waiver Review Division of the Department of State.
