Transfer/Change of Service Site – Contract Suspension
A contract suspension may be requested by loan repayment providers who are unable to complete service at their approved service site but wish to transfer to another eligible service site to fulfill the remaining terms of their contract. The Arizona Administrative Code (A.A.C.) R9-15-217 and R9-15-317 provides the policy for requesting a contract suspension. Once ADHS approves the contract suspension request, loan repayments for services while the contract is suspended shall not be authorized.
If the provider does not obtain employment at an eligible service site at the end of the contract suspension period, the provider will enter into a default status. The appropriate contract provisions for default will be enforced by ADHS.
