First Things First Early Childhood Therapist Incentives Programs
Contract Requirements
If your application has been approved for any of the FTF Early Childhood Incentives Programs, ADHS requires that you provide on a QUARTERLY basis verification of service by submitting the following forms:
- Service Verification Form
(with notarized signature) - Encounter Report Form (with notarized signature)
Note: Obligated service begins on the effective date of the State contract. Services rendered prior to the effective date do not count toward the service obligation.
Certificate of Insurance (COI)
In order to operate, you must have a Certificate of Insurance. Here are some resources to assist with that certification.
