Note: Therapists who would like to be considered must submit all the required forms mentioned above. Only original forms will be accepted. Faxed or electronic submissions will not be accepted.

Required Registration

All applicants must register at the Procure.AZ.gov website.

  • December 16 - March 15
  • March 16 - June 15
  • June 16 - September 15
  • September 16 - December 15

Note: Only complete applications received will be reviewed for consideration of a loan repayment and/or stipend award. All applicants will be notified of ADHS decision within 50 business days.
